The Health Smart
Card is a "cutting edge" technology of smart cards
which uses a computer microchip programmed to store your medical
history and medical insurance data to be read by participating
health care providers.
A printed copy of your
personal data stored on the smart card's microchip is provided
to you with your card, so you can give it to health care providers
who may not yet be participating in the program. Ask them to
make a copy of your information and return to you the original.
Ask them to contact Health Smart Card to obtain a reader for
their office. Included with your Health Smart Card is an adhesive
sticker to place on the light switch closest to the front door
entrance of your home and an electrostatic sticker to apply on
your car window. These stickers will alert emergency personnel
to the fact that you have a Health Smart Card and where it is
located.

If you purchased the programmed smart card without updates included,
then you can obtain any update to the data on your Health Smart
Card for a small fee at anytime. If you purchased a programmed
smart card with all updates included, then all updates to your
Health Smart Card are at no charge for your lifetime.
Whenever a change occurs in your
medical history or insurance information, simply return your
card and the printed copy of your personal data with the changes
recorded, dated, and signed by you with your payment to have
it easily updated at your convenience. The original date and
the update date are recorded on the microchip and printout.
Immediately upon receipt of your
Health Smart Card, please review the original or updated information
for accuracy so that any necessary corrections can be made. By
your use of this card, you agree to accept the responsibility
for the accuracy of all the information stored on the card's
microchip. It is recommended that you keep your card next to
your driver's license.
Participating locations able
to read your Health Smart Card at the time of card issue are
included with materials that accompany your card. Please consider
using these providers. More providers will participate as they
become familiar with their patients carrying the Health Smart
Cards.
