Dentist's Office
The Health Smart Card is a credit card sized card with a computer chip imbedded into it. The Smart Card will carry programmed medical data and insurance billing information of the cardholder. The card is easily updated whenever any changes occur.

By inserting the card into a card reader which connects to a computer via a serial port, the data is displayed on the computer screen in any participating ambulance, emergency department, Dentist's or Doctor's office, pharmacy, outpatient surgical facility, nursing home, etc. The computer screen display is easily printed to paper for inclusion in the chart. Examples of a printout, application form, card appearance and instruction sheet are included.

The benefits to your office are:

1. The private office Dentist will have detailed medical information on the patient immediately, decreasing the time spent filling out medical forms. This will increase efficiency and decrease the time spent getting the patient ready to see the Doctor.
2. If the patients are unable to accurately supply their medical history, for whatever reason, then important medical and drug information is still available.
3. Insurance billing information is provided for up to two carriers with ID numbers and telephone numbers.
4. As the patient's Dentist, your name and phone number can be entered into the card's memory chip along with the patient's medical history.

I am requesting that you place a Health Smart Card reader in your office to be able to read the cards of any of your patients who may come to see you and are participants of this program. If you have a reader in your office, your name will be printed on an instruction sheet which is provided to ALL card holders. This will be free advertising for your office.
